Patriot Hybrid Learning/In-Person Co-Curricular Sign Up Form
For the Second Semester of the 2020-21 school year, Stevenson High School will offer opportunities for students to come to campus in addition to remote learning. In addition, as of late January, students have the choice to participate in clubs and sports in-person. In order to participate in any in-person learning or after-school activities, students (and parents/guardians) must agree to:

      1. wear a mask while on campus.
      2. complete the daily health screener.
      3. participate in the population testing program (more below).
      4. follow social distancing guidelines.
      5. frequently wash hands.
      6. share in the cleaning and disinfection process.

The above efforts are required for any on-campus activities to help mitigate the transmission of COVID on campus.
